The over-the-counter drug brand Imodium, which is manufactured by Janssen Pharmaceuticals Inc. and Johnson & Johnson Consumer Inc., may increase the effect of opioids when consumed at the same time. In September 2019, the FDA approved new package-size and package-type limits for the OTC tablet and capsule forms of the Imodium. The FDA’s action limits the package sizes by limiting each carton to no more than 48mg and provides for unit-dose packaging for the affected products. The FDA believes this could reduce the rate of overdose and death.

The FDA requested that online distributors help mitigate the risks of loperamide abuse by not selling more than one package of the drugs to each customer. The agency also said they are taking steps to ensure that consumers can easily read the product label warnings for drugs before they purchase them.

Loperamide may result in some general side effects, including:

  • Dizziness
  • Drowsiness
  • Dry mouth
  • Stomach discomfort
  • Constipation
  • Nausea

These side effects result from using the drug as directed or prescribed. The FDA has warned that dangerous side effects may occur with overuse or abuse.

There are a number of serious heart-related risks, including:

  • Abnormal heart rhythms
  • Serious cardiac events
  • Long QT syndrome (heart rhythm condition that can cause fast, chaotic heartbeats)
  • Heart electrical cycle disruption
  • Torsades de Pointes (type of abnormal heart rhythm that can lead to sudden cardiac death)
  • Sudden cardiac death
  • Loss of consciousness
  • Cardiac arrest
